About the Form

The Information Services Computer Help Desk 'Install, Move, Add, Change' form (IMAC form) is to be used by administrative units at the University to request work to be done on their computers or printers. This form is primarily used for new computers, new printers, new software, but is also available for work request that pertains to the changing of settings and movement of hardware. This form is only for University owned hardware & software.

Once submitted, the form will be reviewed to ensure all necessary information is contained in the form, and then the form will be approved. Once approved, it will be assigned a work ticket number and placed into call tracking in the order in which it was received. The time & date in which it was faxed will be used to determine what order work requests will be processed.

The IMAC form is for:

  • installing hardware, software, & printers
  • moving computers & printers
  • adding software, drivers, peripherals, settings, etc.
  • changing configuration of hardware, settings of software

The IMAC form is NOT for:

  • troubleshooting problems with your computer or software
  • network work requests
  • guest authorizations
  • non-University owned machines

Making a Request

To place a work request, please complete the IMAC form, print, sign and fax it to x2150. Please complete one form per each individual computer/printer install, move, add, and change. Incomplete forms, or work requests out of scope of Computer Purchase and Service-Policy Number AC-1 or the Managed PCPolicy Number AC-3, will be returned to the requestor for revision.
